Atrium Health Community Cares, Powered by Kohl’s, is a regional health outreach initiative focused on fragile communities within Mecklenburg, Anson, Cabarrus, Gaston, Union and York counties. The traveling health care program will be staffed by an advanced care practitioner, a registered dietitian, an athletic trainer, a registered nurse and will also have regular access to a wide-range of other health care providers within Atrium Health.
A recent community health improvement study conducted by Atrium Health identified core focus areas for improving community health: access to health care, mental health, obesity, tobacco cessation and food security. Atrium Health Community Care, Powered by Kohl’s, will address these core needs by developing a traveling health care team to go into identified communities to provide care ranging from whole child wellness exams, immunizations, sport physicals, food security screenings, mental health screenings, tobacco cessation classes and obesity related medical care coordination.
